The CalFresh Healthy Living (CFHL) goal is to implement policy, systems and environmental change projects along with nutrition and physical activity education interventions for eligible populations by:

  • Increasing consumption and access to healthy dietary choices where people live, learn, work, or play
  • Reducing consumption and/or appeal of unhealthy dietary choices
  • Increase access to and/or appeal of physical activity opportunities
  • Educating the Placer County SNAP-Ed population on healthy choices

Services Provided

  • Nutrition and Physical Activity Education: A variety of nutrition and physical activity lessons and workshops are available to eligible schools, community groups, and organizations. In 2005, Placer County Systems of Care with the support of the Mental Health Services Act (MHSA), joined forces with a host of community partners to launch a campaign to transform mental health services through community-based, recovery focused, culturally competent and innovative approaches. Believing the key to lasting change in mental health care in Placer County is to place residents and community based agencies in a true position of partnership with the county, the Campaign for Community Wellness (CCW) was created.

The Placer CCW functions as the legislatively mandated body to oversee the implementation of the Mental Health Services Act. The CCW is comprised of concerned community members, non-profit agencies, school and law enforcement partners, family members and consumers of Mental Health services and the Placer County Systems of Care staff. Advocacy Services

Advocates meet with clients on a bi-weekly or weekly basis to set goals using a strengths-based, client-centered approach. The program works to connect clients with resources and to assist clients in navigating all GHC services when appropriate, such as counseling, name and gender change assistance, health insurance options, economic empowerment program, hormone clinics and legal health clinics.

Name and Gender Change Paperwork Assistance

Advocates assist clients in filling out name and gender change court paperwork and identity documents, such as driver license, birth certificate, passport and social security changes; become familiar with the court process for changing name and gender; provide information to clients around the process for legal name and gender change.

All products are free and you can register for a unique pin number to access products easily online or in-person.
